Potential Pathogenesis, Diagnostic MRI, and Surgical Findings of Intradural Lumbar Disc Herniation: A Report of Two Cases

Abstract
Study Design: Case report.

\Objectives: This study presents two patients with recurrent intradural lumbar disc herniation (ILDH) and discusses the specific magnetic resonance imaging (MRI) findings, potential pathogenesis, surgical findings, and characteristics of recurrent ILDH.

Summary of Literature Review: ILDH is a rare disease that can cause serious neurologic deficits. Surgical treatment is usually necessary to avoid further neurologic exacerbation.

Materials and Methods: A 78-year-old male patient and a 63-year-old female patient presented with ILDH. Both patients underwent microscopic discectomy and experienced recurrence. The 63-year-old female patient underwent reoperation after the first surgery, but the 78-year-old male patient postponed reoperation due to the risk of surgery from an aortic aneurysm that had been found prior to his initial surgery. Even after reoperation, the 63-year-old female patient complained of newly discovered neurologic symptoms on the opposite leg.

Results & Conclusions: Preoperative MRI findings can help diagnose ILDH and reduce the incidence of neurologic complications through early surgical treatment. They also can help discover recurrent ILDH. The timing of surgery and the time lapse between ILDH formation and detection seem to affect the prognosis of ILDH, and reoperation following recurrence may not yield as satisfying results as the primary surgery.
